Concord Hospital's Diabetes Self-Management Program provides training and self-management education to people with diabetes and their families. The program is a comprehensive outpatient referral-based diabetes education program that empowers you to take control and successfully manage your diabetes. By participating in our program, you have an opportunity to fully understand the rationale for early aggressive intervention to prevent complications from diabetes mellitus.
Self-Management Training

Our diabetes educators work closely with your physician or primary care provider. Physicians receive a summary of the visits and patient specific goals.

Group classes and individual consultations are available for the following:

* Pre-diabetes and prevention of diabetes
* Basics of type 1, type 2 and gestational diabetes
* Blood glucose monitoring
* Nutrition counseling for blood glucose and lipid management
* Guidelines for meal planning, dining out, grocery shopping, label reading and travel
* Insulin and medication
* Diabetes and sick days
* Acute and chronic complications
* Stress management
* Behavior goal setting
* Preconception care and gestational diabetes
* Introduction to pump therapy
* Glucose sensor
* Exercise program based on your fitness level, interest and needs

Group Classes

The more you know about diabetes, the better you can care for yourself. The following group classes are available:

New Beginnings - All newly diagnosed people with type 2 diabetes are encouraged to attend this class prior to the individualized visit. This 1 ? hour starter class provides a brief overview of nutrition, exercise, medications and monitoring in order to get started with diabetes management.

Self-Management - All people with type 2 diabetes are encouraged to attend this class for detailed education. The 4, 2-hour class series includes information on nutrition, exercise, monitoring, medications, complications, community resources and stress management.

Advanced Carbohydrate Counting - This class is recommended for those who wish to intensify their insulin management and for those who are considering insulin pump therapy.

Introduction to Insulin Pump Therapy - This class is offered once a month to those who wish to learn about insulin pump therapy.

The American Diabetes Association recognizes this education service as meeting the national standards for diabetes self-management education.

The program includes dietitians and nurses who are certified diabetes educators. Two of the dietitians are also board-certified in advanced diabetes management by the American Nurses Credentialing Center. Educators conduct a thorough patient assessment to recommend safe and appropriate medication and/or insulin requirements based on pattern management.
