Diabetes Strip Assistance Program
Background:
In the spring of 1997, members of Sheridan Community Diabetes Education Committee wrote two grants to Wyoming Diabetes Control Program. One of the grants was the Strip Assistance Program. Additional funds have been adde from a grant from the Homer A. & Mildred S. Scott Foundation, donations and fund raisers. This program remains in place.

Program:

For persons with diabetes mellitus who test their blood glucose at home, who meet financial guidelines and who do not receive reimbursement from Medicare, Medicaid or insurance for blood glucose test strips would qualify for this program. Persons who test 1-2 x per day or less receive one $20 coupon per month. Persons who test 3 x per day or more receive two $20 coupons per month. These coupons are redeemable at any Sheridan pharmacy for a period of six months. Clients are required to have a Hemogloblin A1c blood test every six months.

HOW TO APPLY:

Call Wyoming Rehab at 674-1632. Allow approximately 15-30 minutes for your appointment. Coupons are mailed to clients by the Community Health office.
