Diabetes and Endocrine Center Services
Our goal is to provide every patient with quality information and the resources they need to manage their diabetes successfully. Harbor's Clinical Care Services, with experienced endocrinologists, provide both inpatient and outpatient care to our patients with diabetes.

Our services include:

* Self-management — Learn the skills and training you need to manage on your own, including glucose monitoring.
* Nutritional instruction — Dieticians are on staff to support your management plan.
* Medication management — Instruction for those needing insulin initiation.
* Educational Programs — Programs available for patients, community residents and healthcare providers. Read more below.
* Individual Educator Appointments — Meet with the nurse and dietitian to learn skills to manage your diabetes, available for women experiencing gestational diabetes. Read more below.

Education Services
Experienced certified diabetes educators, including a registered dietitian and registered nurse teach the program. Certified diabetes educators are health professionals with advanced training in the area of diabetes care. Persons with Type 1, Type 2 and secondary forms of diabetes, including gestational diabetes, can learn skills to manage their diabetes and meet goals set by themselves, their physician and healthcare team.

Individual Educator Appointments
Physicians can refer persons with diabetes to the nurse educator or the dietician educator for individual education sessions. A referral from the primary care physician and pre-approval by the health insurance provider is required. Patients are strongly encouraged to bring a family member or support person to any of our teaching sessions.

At your appointment the Nurse Educator or the Dietician Educator will teach you helfpul tools for managing your diabetes, including:

* Living a healthy life, despite your diabetes
* Controlling your diabetes
* Glucose monitoring
* Preventing and slowing down diabetes complications
* Eating healthy
* Exercising
* Taking your medications properly
* Foot care
* The role of regular follow-up care
* Counting carbohydrates
* Coping with challenges of having diabetes
