Primary Health Care Services
These services are available by self-referral to those at risk for developing diabetes, those living with diabetes and those living with someone with diabetes. The programs vary in length and are taught by a multi disciplinary team of health professionals.

Two 'Day 1' programs per month
One 'Day 2' program per month (optional follow up program)
