Mission: The mission of the Bartholomew County Medication Assistance Program is to improve the health and quality of life of individuals in need of better access to prescription medications.

Many pharmaceutical companies have programs to help people who have long-term medication needs and limited financial resources. The Bartholomew County Medication Assistance Program, working with an individuals physician, helps that person apply for free or low-cost medicine offered by the drug companies.

The program coordinator is a Registered Nurse. The staff identify sources of help for clients, assess eligibility, prepare applications and ensure eligible clients receive needed prescriptions.
Typical medical conditions covered

* Allergies
* Anxiety/Depression
* Arthritis
* Asthma
* Diabetes
* Gastrointestinal Disorders
* Heart Disease
* Hormone Replacement
* Hypertension
* Other Chronic Illness

Eligibility Guidelines

* Receiving medical services from a physician in Bartholomew Co.
* Unable to pay for prescribed drugs due to financial hardship
* Currently under treatment for chronic illness such as diabetes, hypertension, arthritis or asthma
* No insurance coverage to help pay for prescriptions, and ineligible for Medicaid or other public assistance
* Medicare beneficiaries may qualify if they have no prescription drug coverage through another plan
* Meet all drug company eligibility requirements (proof of income and U.S. Citizenship is often required)

The prescribing physician must be willing to participate as required by the drug companies.
How to Apply

* Call 372-6918 to ask for a program application
* Complete application
* Schedule an appointment to discuss your need
* Bring your completed application, proof of income for your entire household, insurance information, prescription information and Bartholomew County physician’s name

For those with extraordinary circumstances, limited help with emergency prescriptions may be available.
Anyone interested must meet the following guidelines

* Be a resident of Bartholomew County
* Have income below 180% of the Federal Poverty Guidelines
* Have no prescription coverage through insurance, Medicare supplement or Medicaid
* Be able to prove residency and income

Both the chronic program and the immediate assistance program are the result of a community collaboration between the Healthy Communities Initiative of Bartholomew County and a number of partners. In addition, local donors have made generous contributions to make these services possible.
