AMFH Program Description
The Alberta Monitoring For Health Program enables Albertans with diabetes, who meet eligibility requirements, to access limited funding to cover a portion of the cost of eligible diabetes management supplies. This program is administered by the Canadian Diabetes Association and funded by Alberta Health and Wellness (under the Healthy Living Branch).
Who qualifies for the AMFH Program?
People wishing to access this program must meet the following eligibility requirements:
* Have diabetes mellitus;
* Are a full-time resident of Alberta;
* Meet one of the following three family status and income levels
o Single with an annual, taxable income less than $23,598.
o Married/Common law with a combined, spousal, annual, taxable income less than $37,021.
o Single/Married/Common Law (with dependant children)with a combined family, annual, taxable income less than $44,974.
* Manage their diabetes through diet, oral medications and/or insulin therapy;
* Have supplies and/or medication prescribed by a physician;
* Do not have insurance coverage for any diabetes management supplies, i.e. blood glucose test strips, injection supplies, lancets and pump supplies. Having insurance coverage for insulin or oral medication will not affect eligibility;
* Have received training in self-monitoring of blood glucose;
* Have valid Alberta Health Care insurance registration

What type of coverage does the program provide?
* People managing their diabetes through insulin will be eligible for up to $550 per year in financial assistance, prorated through the AMFH Program.
* People managing their diabetes through oral medication(s) will be eligible for up to $250 per year in financial assistance (prorated).
* People managing their diabetes through diet will be eligible for a maximum of $100 per year in financial assistance (not prorated).
These benefit categories reflect the importance of the variety of diabetes management regimes in order to avoid or delay the devastating complications of diabetes.
How do I apply for the program?
Applications for the AMFH Program are available at Canadian Diabetes Association offices across Alberta, online or may be available from your local pharmacist or family physician.
How long will it take to know if I have qualified and will receive support?
Approximately two weeks from the time your application has been received by the AMFH Program, a letter stating your approval or denial will be sent to you. Letters of approval will be accompanied by an AMFH Program Benefits Card. Financial support can be accessed once you have received your AMFH Program Benefits Card.
