INFORMATION ON REGISTRATIO
PACKET AND DEADLINE
Registration packets (includes camp fee and
scholarship form) will be mailed after Sunday, April 5,
2009 to those requesting a packet . The camp
fee is NOT established until after the Roll-A-Thon
is held (a fund raiser to benefit camp) . You may send
in the attached request for a registration now and a
registration packet will be sent to you after Sunday.

Space for camp is limited. When you receive the
registration packet (after Sunday, April 5, 2009), we
encourage you to complete and return the
registration packet to our office onor before
Tuesday, Mav5, 2009. Registration
packets turned In after May 5, 2009, or not
fully completed, will only be considered if
space Is available AND there is time to
process the packet.
Space is reserved for newly diagnosed
children who come in after the May 5th.

CAMP FEE, CREDITS Pf SCHOLARSHIPS
Camp fee includes all activities, food, and
diabetes supplies.
Camp Credits. Parents can earn credits to
offset the camp fee . For more information regarding
earning credits to camp, please call our office at
(253)272-5134.
Scholarships . Thanks to the support of our
sponsors, eligible campers with diabetes also can
receive full or partial scholarships . A scholarship
form is sent with every registration packet .
