We are the diabetes centre (DC) that serves Liverpool and surrounding areas. We provide information on all aspects of how to care for diabetes. This is tailored to people with diabetes and their interested family members as well as other health professionals.
We promote self-management to enhance your quality of life and to reduce the risk of developing diabetes complications. We work closely with other community agencies to provide comprehensive diabetes care for our area.
We are open on Wednesday and Thursday from 8:00 a.m. - 4:00 p.m. We also operate our satellite service to the North Queens Medical Centre every 3rd Thursday of the month.
To make an appointment or ask about our services, please phone (902) 354–3175 or (902) 354-3437 ext. 1257. Your first appointment will be made after your doctor has referred you to our program. The staff of the diabetes program will arrange follow-up appointments.
If you have diabetes (type 1 or 2 – adult, child, adolescent, or young adult), are pregnant and have diabetes, or have been told you have a mild form of diabetes, we would be more than happy to assist you in managing your diabetes.
We provide a number of ways to learn about your diabetes. All of these start with an individual appointment where your needs are discussed with the nurse and dietitian. We offer:
* Individual instruction.
* Group instruction. (This provides you with an opportunity to learn from and with others who also have diabetes or prediabetes.)
* Insulin initiation. (If you need insulin, this means that you can start taking your insulin as an outpatient. You may not need to be admitted to the hospital.)
* Survival skills program. (This short, 3-hour session provides answers to commonly-asked questions about your diabetes.)
Other: We accept self-referral for the educational components of our program.
