THE PROBLEM
Rising Prevalance The incidence and prevalence of diabetes continues to grow rapidly. In 2005, 18.5 million Americans were diagnosed with diabetes. In 2007, the number rose to 20.8 million. By 2015, the number is projected to grow to 40 million Americans diagnosed with diabetes.
Rising Healthcare Costs Employees with diabetes have higher healthcare costs than those without diabetes. Annually, an employee with diabetes will cost $13, 243 on average, while an employee without diabetes costs only $2,560 on average.
Rising Absenteeism Employees with diabetes have markedly more absences compared to those without diabetes. Men with diabetes have 11 more absences per year, and women with diabetes have 8.7 more absences per year-both compared with men and women without diabetes.
OUR VALUE PROPOSITION
Our program of care is ongoing. A patient will see us 5 times a year, each time visiting with the physician, educator/dietitian, and getting lab results.
We offer our coordinated and comprehensive program at our full-service outpatient facilities. We currently have 11 Health Centers across Texas-in Houston, Dallas, San Antonio, and Corpus Christi.
We use Electronic Medical Records for our population management and reporting functions. EMR allows us to easily show reports and outcomes of our program.
We focus on engaging the patient in his/her own diabetes management, promoting diabetes wellness, and preventing complications.
SAVINGS TO EMPLOYERS
Studies have shown that when diabetes is well controlled, health care costs go down. This means employers and health insurance companies pay less money for the care of their employees. Also, the better controlled a diabetic is, the lower his/her risk is for developing long-term complications from diabetes-complications that are very expensive to treat, like retinopathy (eye disease), nephropathy (kidney disease), and neuropathy (nerve disease).
