Education Program
Taking Care of Diabetes Does Make a Difference………
Studies have shown that keeping diabetes well controlled can help prevent complications such as diseases of the heart, eyes, kidneys, and nerves. Diabetes is primarily a self-managed disease which means that the individual takes responsibility for day-to-day care. The multidisciplinary team of diabetes educators at the Fort Sanders Diabetes Center provides training and support to help meet this challenge.
The Fort Sanders Diabetes Self-Management Education Program
The staff at the Fort Sanders Diabetes Center can help people learn how to manage type 1, type 2 and gestational diabetes. Working with a team of diabetes educators, which includes dietitians, nurses, exercise physiologists, and a counselor, participants develop an individualized self-management plan. During the program they learn how to balance meals, physical activity, medication, and blood sugar monitoring routines. The educators counsel participants on reducing the risk of diabetes-related complications, enabling them to problem-solve and develop healthy coping strategies. The Fort Sanders Diabetes Center also offers monthly support groups which are open to patients and their family members.
Referral Process
American Diabetes Association The diabetes self-management education program is offered during the day and evening. Most health insurance plans, including Medicare, provide coverage for diabetes education. The staff at the Fort Sanders Diabetes Center will provide assistance with the referral process. The program at the Fort Sanders Diabetes Center has been recognized by the American Diabetes Association (ADA) for Quality Self-Management Education.
