Medication Bridge Program is a private-public partnership under the leadership of the Foundation for Healthy Communities to help eligible uninsured and under-insured patients of all ages to receive needed medications from pharmaceutical companies' Patient Assistance Programs. Gift of Fitness - Through a partnership with the YMCA, we offer three month complimentary introductory memberships at a patient's local YM/YWCA. Exercise can greatly improve outlook and outcomes for kidney patients and transplant recipients. Referrals come from social workers, nurses and physicians at area dialysis and transplant centers. In some cases, patients can also arrange for discounts on continuing memberships.
