SoonerPlan
SoonerPlan is Oklahoma’s family planning program for uninsured women and men who are not enrolled in regular SoonerCare services.
Eligibility
* Age 19 or older;
* U.S. citizens or qualified aliens;
* Oklahoma residents;
* Not enrolled in regular SoonerCare;
* Without creditable health insurance coverage;
* Within the income standard (185 percent of Federal Poverty Level).
Creditable coverage includes Medicare and other Health Insurance. A stand alone Dental, Vision or Pharmacy Plan is not considered creditable coverage.
Services
You may receive family planning services from any SoonerCare provider who offers family planning. SoonerPlan will pay for the following services:
* Birth control information and supplies;
* Laboratory tests related to family planning services, including pap smears and screening for sexually transmitted infections;
* Office visits and physical exams related to family planning;
* Pregnancy tests for women;
* Tubal ligations for women age 21 and older;
* Vasectomies for men age 21 and older.
