The Tri-Hospital Diabetes Education Centre (TRIDEC) was founded in 1971 by health-care professionals from the Women's College, Toronto General and Mount Sinai Hospitals, to provide diabetes education in an ambulatory setting. TRIDEC has served as a model for the development of diabetes education centres across Canada, played a leadership role in the training and education of diabetes healthcare professionals and worked with community groups to support local education initiatives.

TRIDEC offers a range of individual and group diabetes education programs, which include counselling from a multidisciplinary team. We work with our clients to help them identify needs and learn the necessary self-care skills to manage diabetes. Relatives, friends and other support people are encouraged to attend.

Appointments can be made with a nurse, dietitian or social worker to discuss on-going diabetes self-management issues.

Individuals age 16 and older with type 1, type 2, pre-diabetes and pregnant women with diabetes are eligible to participate in the program.

Referral
A referral from a physician is required.

Fees
The cost of the program is covered by Provincial Health Care Plans. There is a small fee for meals and materials during day programs.
