If you are one of the 70,000 people in Delaware with diabetes. you do not have to deal with your disease alone! The Delaware Diabetes Coalition is a non profit organization comprised of volunteers from all corners of our state. We include doctors, nurses, dietitians, pharmacists, educators, representatives of managed care organizations, a utilization review organization, Public Health and other non profit organization such as the American Diabetes Association, The Juvenile Diabetes and Research Foundation, and the American Heart Association. These community and professional representatives want you to receive care according to the diabetes national standards so you can live a healthier life with diabetes.

The mission of the Coalition is to improve the quality of life for people in Delaware affected by diabetes through awareness, prevention, identification of services and advocacy. This website is designed to be informative and to provide you with a valuable resource tool. The Delaware Diabetes Coalition publishes The Community Resource Guide for persons with diabetes which includes statewide resources for medical, nutritional, and professional services as well as educational and financial assistance. The information included will help you to know what questions to ask both your primary care provider and your insurance company.

Who We Are

The Delaware Diabetes Coalition, Inc. is a statewide nonprofit organization comprised of a network of health care organizations working together to improve the lives of all people in Delaware affected by diabetes through awareness, prevention, identification, dissemination of services and advocacy. The Coalition was established in 1994 and received non-profit recognition in 2001.

The Coalition takes an active role in the development, support and advocacy for standards of care, public policies, and comprehensive plans for the prevention and control of diabetes throughout the state of Delaware. The Coalition encourages and promotes improvements in diabetes based statewide systems with regard to awareness and prevention, early detection, patient care and prevention of secondary complications of diabetes. The Coalition also encourages and promotes public and professional education programs with regard to awareness, prevention and early detection of diabetes and its complications.

Some of the activities of the Coalition include:

The annual Diabetes EXPO, a public event held in November each year, this year to be held at the Sheraton Dover Hotel, Wednesday, November 14th, 2007, 10:00 am to 3:00 pm.

The publication and updates of several resource guides, The Community Resource Guide, for individuals, a professional edition for Health Care Professionals, and Diabetes in the Workplace for employers.

Collaboration with several third party healthcare organizations to standardize flow sheets for documentation in patient charts by primary care providers.

Collaboration with Quality Insights of Delaware a utilization review organization.

Collaboration the with Medical Society of Delaware, Podiatric Society, and Ophthalmology Society to develop standardized forms regarding documentation of services for prevention and treatment of complications related to diabetes.

Working with the Delaware Pharmacy Society to do secondary screenings for complications and education for individuals known to have diabetes, who are out of control based on their A1c findings.
